The University is arranging the Department of Health Human Papillomavirus (HPV) Vaccination Catch-up Programme at CUHK campus. CUHK students who fulfill the enrolment requirement of the programme and are HK identity card holders are eligible to join.

Female Hong Kong residents studying undergraduate/ postgraduate programmes in the Chinese University of Hong Kong who were born between 2004 and 2008, and have not yet completed two doses of HPV vaccination

Application Method and Procedure:
Pre-registration is required with limited quota, filled on first-come-first served basis. On-site registration will not be accepted.

Important Information:
(1) Participants must present valid and original Hong Kong Identity Card and CUHK Student ID Card on the day of vaccination. Individuals with an expired student ID card should arrange vaccination under the Human Papillomavirus (HPV) Vaccination Catch-up Programme (Phase 3).
(2) Participants must agree to and register for the use of eHealth as part of the programme.
(3) Under the Human Papillomavirus (HPV) Vaccination Catch-up Programme, each eligible individual will receive two doses of the vaccine free of charge. Immunocompromised individuals will require three doses (all free of charge) and must present a valid written referral from a registered doctor as proof.
(4) Individuals who have received two doses of the HPV vaccine before are deemed as completion of HPV vaccination and therefore will not be eligible to participate in the Catch-up program.
(5) Participants should consent to vaccination on the day of vaccination. For participants under the age of 18 , the “HPV Vaccination Consent Form” must be signed by their parents or guardians. Please download the form in advance and submit the signed form on the vaccination day.
(6) Please bring the vaccination record (vaccination card) on the day of vaccination for verification by vaccination team. Participants applying for the second HPV vaccination must present their first-dose HPV vaccination record to the vaccination team for eligibility verification.
(7) Participants are required to arrive at the vaccination venue 5 minutes before their scheduled time slot.
(8) Incorrect or incomplete documentation of the required information may result in denial of registration and vaccination. The vaccination team reserves the final right to verify eligibility.
