Primary Care & Older Persons – Key to Medical & Social Integration

Older persons, particularly those with chronic diseases, have complex needs requiring medical as well as social care to be provided in a seamless manner. Primary care can be such a platform for health and social care providers for management of chronic diseases, psychosocial problems, as well as health promotion.

This symposium provided a forum where health and social service providers, academia and experts jointly deliberated models and strategies in building a primary care system for older persons in Hong Kong.

Two distinguished overseas speakers:
  • Professor Steve Iliffe (Professor of Primary Care for Older People, University College London)
  • Professor Barbara Starfield (Professor of Department of Health Policy and Management, Johns Hopkins Bloomberg School of Public Health)
