長者與基層健康 ─ 醫社合作之契機

Older persons, particularly those with chronic diseases, have complex needs requiring medical as well as social care to be provided in a seamless manner. Primary care can be such a platform for health and social care providers for management of chronic diseases, psychosocial problems, as well as health promotion.

This symposium provided a forum where health and social service providers, academia and experts jointly deliberated models and strategies in building a primary care system for older persons in Hong Kong.

Two distinguished overseas speakers:

Professor Steve Iliffe (Professor of Primary Care for Older People, University College London)
Professor Barbara Starfield (Professor of Department of Health Policy and Management, Johns Hopkins Bloomberg School of Public Health)
