預設照顧計劃的執行科學
預設照顧計劃讓病人可以與家人及醫護人員一起，就日後的護理安排釐清想法和意向。然而，這計劃的執行過程或會牽涉各種社會文化因素，從而影響成果。我們的研究嘗試運用不同策略，促進醫院、院舍、家庭及社區護理機構之間的溝通，並探討有關運作機制。我們亦編製了各式各樣的教材，以提升公眾的關注。我們的研究成果對醫護發展的貢獻主要在以下三方面：
- 建設社區能力以推行預設照顧計劃；
- 制訂相關指引及政策以推動預設照顧計劃的實踐；及
- 建立讓患者更易於獲得預設照顧計劃及持續照護的護理模式。
