COVID-19 PCR Test
Terms and Conditions and Personal Information Collection Statement
General
1. There are possibilities of false positive or screening error due to insufficient specimen or other reasons. A negative COVID-19 PCR test result does not preclude you from contracting any disease, including COVID-19. A negative COVID-19 PCR test result is not conclusive evidence as to your health status. You shall not rely on such negative COVID-19 PCR test result to self-diagnose or to self-treat any health-related conditions. You should consult a qualified medical practitioner for medical advice and/or treatment.
2. For specimens provided for COVID-19 PCR test, you warrant and confirm that the specimen belongs to you who is the person booked for the test. It shall be a criminal offence to provide false or misleading information.
3. CUHK Medical Centre Limited (“CUHKMC”) does not accept health care voucher under the Elderly Health Care Voucher Scheme as payment for the fee for COVID-19 test service.
4. Additional fee may be levied if you request the re-issuance of the lab report for whatever reasons, including incorrect information provided by you at the time of online booking. The lead time for the re-issuance of lab report is at least one working day.
5. Please note that the fee for COVID-19 PCR test may not be reimbursed by insurance company. You should consult your insurance company directly to ascertain your insurance coverage prior to receiving the COVID-19 PCR test. CUHKMC shall not be responsible or be held liable for any such irrecoverable testing fee.
6. If your condition falls within the scope of the Guidelines of the Centre for Health Protection (CHP), CUHKMC is required to report to CHP and contact you immediately.
Online Booking
7. You have to make online booking for the COVID-19 PCR test. CUHKMC will send you SMS confirmation upon successful booking for the COVID-19 PCR test.
8. Once the online booking for the COVID-19 PCR test is confirmed, no rescheduling or cancellation will be accepted. The booking is non-changeable, non-transferable and (if applicable) non-refundable.
9. If you fail to attend to receive the testing services at the scheduled appointment time for any reasons, whether with or without notice to CUHKMC, the appointment will be forfeited without notice and CUHKMC will not refund any monies paid and will under no circumstances be liable for any costs or expenses arising therefrom.
10. Substitution of client is strictly prohibited. CUHKMC will verify your identity with the booking name and other information provided in the online booking system to ensure the testing service is provided to the correct person. CUHKMC may disallow any person who is not the person recorded in the online booking system to receive the testing services without any refund.
Personal Information Collection Statement
11. In order to book online, you will be required to provide CUHKMC with your Hong Kong Identity Card number, date of birth, e-mail address, and local mobile phone number which can receive SMS. Personal data collected from you or about you (including COVID-19 PCR test related information) will be used by CUHKMC for processing and confirming the booking, verifying eligibility for COVID-19 PCR test, test result notification, purposes relating to your healthcare and/or generally for medical care or treatment purposes, facilitating the work of the Government in controlling the spread of COVID-19 and any purposes directly relating to any of the foregoing.
12. Personal data collected from you or about you (including COVID-19 PCR test related information) will be stored as part of your medical records maintained by CUHKMC. It is not mandatory for you to provide the personal data as required, however, if the required personal data is not provided or that the personal data provided is inaccurate, incomplete, or false, CUHKMC may not be able to provide the requested service(s). CUHKMC reserves the right to review the booking and, where appropriate, reject or cancel the booking if any information provided is invalid, inaccurate, incomplete, false or misleading.
13. CUHKMC may also transfer your personal data to the Department of Health, other relevant authorities and organisations collaborating with the Government for the purposes aforementioned.
14. You have the right to request access to and/or correction of your personal data held by CUHKMC. You have to make your data access or correction request in writing to our Data Protection Officer by post to Medical Records Office, CUHK Medical Centre, 9 Chak Cheung Street, Shatin, New Territories, or by email to mro@cuhkmc.hk. Any data access request is subject to payment of a reasonable fee.
Other Terms
15. Before proceeding with the online booking, you should read CUHKMC Disclaimer and Privacy Policy carefully.
16. In case of any dispute, CUHKMC’s decision shall be final and conclusive. CUHKMC shall have the final decision to not provide the testing services.
17. If there is any inconsistency or conflict between the English and Chinese versions of these Terms and Conditions and Personal Information Collection Statement, the English version shall prevail.
