INTRODUCTION

Team 1 is the Hepatobiliary and Pancreatic Surgery Division of the Department of Surgery in Prince of Wales Hospital, which is also the teaching hospital of the Chinese University of Hong Kong. Currently it consists of two professors, one clinical professional consultant two consultants, three associate consultants, one specialist residents, three surgical trainees and two surgical interns. It also has one research assistant. The main operation area consists of wards 8A, 8B and 8C. Day case patients will be admitted to ward 10LM. Patients may also be cared in intensive care unit or private ward as appropriate. The outpatient Team 1 clinic is at 2/F North Wing of Li Ka Shing Outpatient Clinic. Most operations would be carried out in Prince of Wales Hospital while in some selected cases, operations will be done in Alice Ho Miu Ling Nethersole Hospital as day surgery or short stay surgery. Apart from providing clinical service to general surgical patients and patients with hepatobiliary and pancreatic diseases, Team 1 also provides teaching to medical students of the Chinese University of Hong Kong and carries out both clinical and scientific research.

SERVICES

Team 1 is one of the three general surgery teams within the department. Like other general surgery teams, Team 1 deals with general surgical cases like hernia, cutaneous lumps and bumps. It also receives emergency surgical cases such as acute appendicitis and perforated peptic ulcers. On the other hand, Team 1 needs to take care of patients with surgical diseases of the liver, biliary tract and pancreas in the New Territories East Cluster.

The main bulk of elective work of Team 1 involves the treatment of liver malignancies in terms of liver resection or local ablation therapy. It also collaborates with the interventional radiologist for transarterial therapy for primary liver cancer. In addition to simple cholecystectomy, Team 1 also deals with complicated gallbladder and biliary tract diseases like choledochal cysts, Mirizzi Syndrome and bile duct tumors. Surgical treatment of pancreatic tumors is also included in our service with predominately pancreaticoduodenectomy and distal pancreatectomy as our major scope of work.

Team 1 is also involved in endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic ultrasound (EUS) as diagnostic and therapeutic endoscopic procedures provided to our patients.

Collaboration with other specialties:

  • Joint Hepatoma Clinic - a multidisciplinary approach with oncologist and radiologist for care of hepatoma patients.
  • Combined meeting with hepatologist, oncologist and radiologist for research and inter-department collaboration project.
  • Combined pancreatic cancer meeting with gastroenterologist, radiologist and oncologist on management and research on pancreatic cancer.

ACHIEVEMENTS

  • Establishment of a liver function assessment center to facilitate the pre-operative workup and assessment for patients undergoing hepatectomy
  • Establish the use of no-clamp hepatectomy with low central venous pressure as the standard liver resection technique
  • Development of minimally invasive approach for liver resection: laparoscopic and robotic hepatectomy
  • Development of new local ablation technology for liver tumors: radiofrequency and microwave ablation
  • Adoption of pre-operative portal vein embolization as an adjunct for patients with small future liver remnant
  • Adoption of fibroscan as a pre-hepatectomy liver function assessment tool
  • Development of minimally invasive surgery for pancreatectomy: laparoscopic and robotic approach
  • Development of single incision laparoscopic cholecystectomy
  • Maintain a low mortality and low morbidity for both hepatectomy and pancreatectomy
