Implementation science of advance care planning

Advance care planning enables individuals to clarify values and preferences for future care with family members and healthcare providers. However, its implementation process may be related to various factors in the sociocultural context, and thereby affecting the outcomes. Our research attempts to use various strategies to facilitate the conversation across hospital, care home, community and home care settings and explore the underlying mechanism. We have also developed different types of educational materials for raising public awareness. Our work contributes to nursing and healthcare in three aspects:  

  1. building capacity for facilitating advance care planning;
  2. formulating guidelines and policies to support the practice; and
  3. developing model of care to improve access to advance care planning and continuity of care.
