Final Reminder: Verification and Update of Personal Information for Graduation Records |

Dear Graduating Students, |

Following our e-mail notification dated 22 April 2022, this is to remind you once again to check / verify your personal particulars under CUSIS (specifically your registered name BOTH in English and Chinese, where applicable). The registered name recorded under CUSIS will be printed on your completion transcript and letter of certification which will be sent to your HOME address after your graduation is approved by the Senate, and on your degree certificate(s) which will be available for collection in October/November 2022. |

Please note that in accordance with University Regulation (Clause 1.4), “A student shall register in the name which appears in his/her Hong Kong Identity Card or passport. Only under very special circumstances may an applicant who has been recommended by the Undergraduate Examinations Board for graduation or has withdrawn from the University apply for change of his/her registered particulars. Such an application shall be made in writing with justification for approval by the Registrar.” |

If you need to change your registered name under CUSIS, you should submit in person the application form with documentary proof to the Registration and Examinations Section no later than 8 July 2022 (for MBChB students, to the Faculty and Planning Office of the Faculty of Medicine). Please also make sure your HOME address record under CUSIS is correct, and if necessary, to update it on 8 July 2022 or BEFORE, since you will NOT receive the completion transcript and other related documents if the HOME address record is NOT correct. |

Once again, please note that the personal data captured during your undergraduate studies (e.g. name in English and Chinese, home address record, etc.) will be converted as alumni data for the use of the Alumni Affairs Office of the University in accordance with appropriate guidelines, after the approval of the University Senate for graduation records. |
