Admission Requirements

1. Registered Medical Practitioners, Registered Nurses, Registered Chinese Medicine Practitioners, Registered Social Workers, and Enrolled Nurses, Physiotherapists, Occupational Therapists etc. registered with their respective professional bodies or councils; have at least one year of post-registration work experience.
2. Community workers such as Activity Officers, Personal Trainers etc. and have at least two years of work experience in related geriatric fields and a relevant bachelor's degree awarded by a recognized university.
3. Nominees from Non-governmental Organizations and organizations which provide primary care services.

Shortlisted applicants might be invited to attend a face-to-face interview.

Enrolment would be made by filling out "Enrolment Form for Certificate and Diploma / Professional Programmes". Please complete the enrolment form and send copies of supporting documents to one of the Enrolment Centres. Applications will be processed only upon receipt of the completed enrolment form and copies of supporting documents.

Introduction

The programme is intended to provide learners from different disciplines with advanced knowledge and skills to become healthcare planners for older adults in achieving healthy ageing using a case management approach to provide a holistic, multidisciplinary, and evidence-based practice in primary health care.

Learning Outcomes

Upon completion of the programme, students should be able to:
1. Discuss the principles and concepts of primary health care;
2. Discuss concepts of community health assessment;
3. Identify the key elements of planning and evaluation in the development of health promotion projects;
4. Apply evidence-based interventions of primary health care for older adults and caregivers;
5. Work with the stakeholders in the development of primary care strategies for older adults; and
6. Describe the roles of multi-disciplinary team in primary health care.

Delivery Mode

Lectures, Workshop, Seminar, and Compulsory Site Visits
Lectures may take the form of a mass class spread over 5 sessions, or a smaller class over 18 sessions in the evening to give participants more options and greater flexibility in scheduling their training commitments.

Assessment

1. Students are expected to have 80% attendance for the theoretical components and 100% attendance for site visits; and
2. A passing grade in the class presentation.

Award

A Certificate of Completion will be issued to participants who have an attendance of not less than 80% and passed the assessment of the programme.
