Application Form for CPE Helper

Guidelines:

1.All students of Department of Social Work (CUHK) are qualified to apply for CPE student helper.
2. Student helper must attend ALL sessions of the applied course.
3. Successful applicants will be informed via e-mail by 7 days before the start of course date. Otherwise, applications should be assumed as unsuccessful.
4. Successful applicants are required to attend a short briefing session before the commencement of the course.
5. A 5-day prior notice is required for any expected absence or withdrawal.
6. Each student could participate CPE course as student helper no more than twice per academic year.
