Limited no. of seats is available for each workshop. Registration is on first-come-first-served basis.
*Workshop participants must register FOCUS Main Congress on 17-18 November 2018 EXCEPT those register for Midwifery workshop. Workshop fees include coffee/tea breaks.

    (For Hong Kong delegates only.)
  • Credit Card: Visa / MasterCard
    (Please contact your credit card company to notify them of this international charge to prevent the transaction from being rejected.  The merchant name shown on the credit card statement is “ICC LTD” and the amount to be charged is in Hong Kong dollars.)

Rules and Regulations (Registration):

  1. Each registrant should complete a separate registration form. Photocopy of the registration form is acceptable.
  2. Registration form without payment will NOT be processed. Please do NOT send cash.
  3. The Congress Secretariat will send a letter of confirmation by email upon receipt of your registration form and full payment. Kindly check the letter of confirmation.  Any changes or alterations must be made in writing to Congress Secretariat.
  4. Refund for cancellation of registration will be made and subject to the following deadline and administration charge. All cancellations must be made in writing to the FOCUS 2018 Congress Secretariat and the refund will be made after the meeting. Cancellation on/before 18 October 2018, 50% of the registration fee will be refunded. Thereafter, No refund will be made.
  5. A replacement or transfer is allowed provided the replacement is from the same organization and upon receipt of a written request from the person to be replaced. Please note that substitution is permitted on the basis that written notice is received on/before 18 October 2018.
  6. The program is subject to change without prior notice. In the event of cancellation of the Congress, the only liability of the Organizers is to refund all the fees paid.
  7. All enquiries, changes and cancellations should be made in writing to Congress Secretariat.
  8. All the payments are in Hong Kong Dollars only.
