CUHK Convocation Outstanding Service Awards 2018/19

Application Deadline: 28 December 2018 (Friday) 5:00 p.m.

The "CUHK Convocation Outstanding Service Awards 2018/19" are now open for application. The College would nominate students to compete with nominees from other Colleges and Faculties for 2 awards of $10,000 each.

Requirements

Category 1: Outstanding Community Service Award

- Full-time year 2 or above undergraduate students of any disciplines

- Active and enthusiastic participation and promotion of community services

- A personal statement in English under 500 words elaborating on your services and contributions.

Category 2: Outstanding Campus Services Award

- Full-time year 2 or above undergraduate students of any disciplines

- Active and enthusiastic participation and promotion of campus-wide, college-based, or departmental extra-curricular or academic activities

- Outstanding contributions to the University, College or Department

- Outstanding leadership and organizational abilities

- A personal statement in English under 500 words elaborating on your services and contributions

Supporting documents

After completing the application form, please submit the below supporting documents to the Dean of Students' Office before the application deadline:

- A completed application form

- A letter of recommendation addressed to the donor CUHK Convocation (Attn: Mr. Enders Lam) on an appropriate letterhead paper

- Academic transcript from undergraduate year 1 to the second term of 2018/19 (Downloaded copy is acceptable, provided that it has been certified as a true copy by the Faculty with stamp)

- A photocopy of academic transcripts with Explanation Notes of Grading System on an Exchange Programme (if applicable). Any downloaded transcripts are not acceptable

- A personal statement in English under 500 words elaborating on your services and contributions 

- Photocopies of awards received (if any)

- Photocopies of proofs of participation in extra-curricular activities, campus and community services
